Learn how to add new entries to your survey and how to generate a link or QR Code for your survey.
⚠️ Please note that an individual link is created with a new survey entry. Please ONLY USE the general link to distribute the survey. You can find out how to access it further down in this article. |

![]() | Click on this symbol in the survey module to generate the survey link and copy the link to your clipboard for sending it directly to participants | |

Now you can download the QR code and place it anywhere you like (e.g. on your product, flyer, website etc.) to reach as many participants as possible. | ![]() |
⚠️ Keep in mind that the link and the QR code are uniquely made for the usage from only one device in order to give the participants the opportunity to close their browser window, click on the link again and still see their last data entries. This is only possible if the settings in the used browser of the participant are for cookie tracking activated and no incognito window e.g. is used for opening the survey. |
If you want to use a new survey entry also on the same device, then this is the way to go: 1) copy the survey link from the survey 2) add the following ending after the letter-number-combination: /create_new_survey_entry?forceNewCreation=true&app=patient This link creates a new survey entry, no matter if it was clicked from the same device before. |
Third Option (only possible with User Account for Climedo):
Please go to your survey overview by either clicking on the big globe or "Open". | ![]() |
![]() | While in the survey overview you can click on "Add new survey entry" and a new browser tab or window will pop up to fill out the survey. |
⭐ Regardless which way you choose for survey entry, an individual SIC (Survey ID) will be created automatically for each entry. |
